WARRANTY AND RETURN GOODS

Warranty
All products are warranted to be free from defects in workmanship and materials.

Remedy
Any product that proves defective in workmanship or materials will be repaired, or replaced at our discretion free of charge, pursuant to our RGA policy.

Limitations
Repair, modification or alteration to any product that is performed by persons not authorized by Olsen Medical will result in immediate loss of warranty. Defective products must be returned to Olsen Medical, no credit or replacement will be given for any product not returned.

Exclusion
Except for the express warranty stated herein, Olsen Medical disclaims all warranties, expressed or implied, including the implied warranty of merchantability and fitness. The stated warranty and remedy provided are exclusive and in lieu of all other warranties and remedies provided by law.

Limitation
Olsen Medical shall not in any case be held for direct, indirect, special, incidental or consequential damages arising from breach of warranty, breach of contract, negligence, or any other legal theory. Such damages include but are not limited to; loss of profits or revenues, loss of use of the product or any associated equipment, cost of capital, cost of any substitute product, facilities or service, down-time costs, and/or claims of customers for such damages.

Return Goods
Prior permission must be obtained from Olsen Medical Customer Service Department. Upon granting permission, Olsen Medical will forward an RGA Number. This number must be referenced on the outside of every box and on every document sent to Olsen Medical concerning the product return.

• Sterile products must request RMA within 60 days of the invoice date.
• Non-Sterile products must request RMA within 90 days of the invoice date
• All products must be returned within 30 days of the RMA issue date.

Permission to return merchandise should not be interpreted to mean that credit has been given. Olsen Medical reserves the right to hold approval on all returned merchandise until our Quality Assurance Department has completed their inspection.

Merchandise returned without a RMA Number may be returned to the customer, FOB Louisville.

RMAs are valid for 30 days only, product must be received within 30 days or the RMA being closed.

Credit will be issued in accordance with the following conditions and only after inspection by the Quality Assurance Department:

• Merchandise must be returned in a resalable condition.
• Sterile product must be in original, unopened packaging.
• Non-Sterile product must be unused and never sterilized.
