ORDERING INFORMATION

Inside the United States - Olsen Medical uses a network of professional, highly trained distributors, each with a defined area of distribution. Orders should be submitted to the distributor in your area. See the National Distributor map for the distributor in your area, or contact our Customer Service and we will help you contact the correct distributor.

Outside the United States - Orders should be faxed to 502-772-4282 or sent by email to info@olsenmedical.com; orders will not be accepted by phone. The minimum order amount is $250.00 (US), a servicing fee of $150.00 (US) will be invoiced where legalization of documentation is required. All payments must be in US Dollars.

All Orders:

• Product Number - Use the complete product number as shown on this website or our current catalog. There was a change to the product numbers a few years ago, old 3-digit product numbers can no longer be used. Orders with incorrect or incomplete product numbers will be held until confirmation is received.
• Quantity - Products are sold in the unit of measure shown in the current catalog and this website only. Products sold by the case or box are sold as full cases or boxes only. Olsen Medical does not sell partial cases or boxes.
• Shipping Instructions - Unless otherwise indicated, all orders are shipped UPS Ground within the Continental United States, and UPS Expedited for international orders.
• Payment - Terms of payment are set up individually; the amount of credit available is subject to the discretion of Olsen Medical based on information provided by you and verified by us.
